I've used the 1st generation of this mouse until it fell apart (literally), but it took about four years and I got it used on eBay for $10. The 1st gen does what it advertises and saved my wrist. The wireless version is not only overpriced, it's a piece of rock that weights ALMOST HALF LBS. Its weight not only increases the pain from carpal tunnel, its slippery material makes it impossible to lift up the mouse for reposition cursor without triggering the back button. Wireless version is a total design fail, but the wired version is just perfect (it's expensive for a mouse, but cheaper than doctor's bills).
Teflon mouse pad helps, but lifting the mouse is still an issue.
Update 8/30: After almost a month, I'm very disappointed with this mouse. Teflon mouse pad (around $30) solves the rock pushing issue. The tracking is pretty poor for precision work such as CAD, Photoshop..etc. It's alright for general usage.

i have had the evoluent wired mouse for 4 years and it is one my most favored products in my life. I got it while in architecture school when I was using my mouse at least 15 hours a day, clicking, clicking all day long, and developed wrist, elbow, shoulder and back problems. the wired version of this mouse made a huge difference immediately because my arm was not twisted.
HOWEVER: I had to buy an additional one because now I have two workstations, so I thought I would give the wireless one a try. the problem i found is that the 2 AA batteries it requires can make it feel heavy, and I found I had to push it around a lot, which hurt my wrist and arm, ironically! so I ditched it within a week and ordered the wired one. it came today and I am so very happy to have the lightness that the wired mouse has.
maybe if you have a ton of upper body strength this will not be an issue for you.

I've been using the Evoluent Right-Handed Mouse for about 1 month now and really like the feel of it. It definitely reduces the stress on my elbow by keeping my hand in a "neutral handshake position" as advertised. I would recommend it to anyone having elbow pain issues from mousing a lot. I will be adding a split keyboard to my setup soon as well. Try taking your hand(s) off of the mouse/keyboard while you think too...I find that helps a great deal too. I gave the mouse 4 stars as it feels great and only takes a little getting used to. It's a bit harder to do small, precise cursor placement with this mouse. This is because of the handshake position you are using the bigger muscles in your arm, but that is the whole point...to take stress off the little tendons/muscles. If they made it a little lighter in weight that might help. Otherwise I would have given it 5 stars, but I don't call anything "perfect" unless it is. The biggest thing to get used to with the Evoluent is that you can not pick up the mouse to place it on your pad...you have to tilt it onto its side and slide it. This is by design and works well, but takes a bit of getting used to after 20 years of mousing the other way! You definitley have to set the speed of the mouse tracking slower to allow for your bigger arm muscles to accurately move the mouse.

I love it.. i get good tracking, but I also have the usb port mounted on top of my computer with a usb hub. It is sensitive enough for me to use a loose grip and with the handshake position. My mouse also came with an off switch for battery conservation. It only took me about 5 minutes to get used to using it. Top button is left click bottom is right click and scroll wheel is self-explanatory. I didnt even bother installing the software, windows 7 recognized it right away and I dont see anything wrong with the default configuration.
I use the logitech g9 for gaming but this thing seems to have just as many buttons so I dont see any reason why you couldnt use it for gaming also. However, I have not tried as I mostly game on the xbox 360 now (which also has beautiful ergonomic controllers).
Also nice solid feel, easy to press buttons, and look. 80$ well spent.
